Becoming an eye donor is an easy and simple process. Once you have decided to make a difference in someone's life by giving the gift of sight, simply download and complete the consent form below. After you complete the form, call the Alabama Eye Bank at our toll-free number (1-800-423-7811) to inform us of your decision to become an eye donor. One of our friendly and knowledgeable staff members will guide you through the rest of the process and answer any questions you may have.

Financial Contributions

The Alabama Eye Bank recognizes that because of certain medical conditions or personal preferences, you may not choose to become an eye donor However, you can still make a difference by helping us pay to make miracles happen.
